*Plans F and G provide a high-deductible plan in different states. You are required to pay for Medicare-covered expenses under this plan, including coinsurance, copayments, and deductibles up to a $2,490 cap in 2022.

**For Plans K and L, the Medigap plan pays 100% of covered services for the remainder of the calendar year once you reach your annual out-of-pocket maximum and Part B deductible.

***Plan N With the exception of a copayment of up to $20 for select office visits and a copayment of up to $50 for emergency room visits that don’t end in inpatient admission, Plan N covers 100% of the Part B coinsurance.
